If you wish to have your membership card reissued due to loss, etc., a reissue fee of 1,100 yen (tax included) will be charged.
*A separate payment fee will be charged. Please note that this must be paid by the customer.

Please note that your reissued membership card will be delivered the month following the month in which payment of the reissue fee has been confirmed.
